Band 8a, Permanent, Full‑time 37.5 hours per week, Contract: Permanent, Salary £57,528 – £64,750 per annum, Closing 24/06/2026.

Job overview

Lead specialist clinical pharmacist responsible for delivering direct patient care to renal in‑ and out‑patients, managing the renal pharmacy service, and providing expert leadership across the renal medicine directorate.

Key responsibilities

  • Provide clinical pharmacy services in the renal unit, including ward‑based and outpatient pharmacy services.
  • Participate in consultant‑led renal ward rounds, multidisciplinary meetings and ward rounds.
  • Conduct medication reconciliation, review and counselling on high‑risk medications.
  • Identify and support patients needing medication optimisation and early discharge.
  • Liaise with community and care‑home colleagues to support medicine adherence.
  • Contribute to clinical governance, risk, audit, education and teaching within the directorate.
  • Manage junior staff, provide undergraduate or junior pharmacist supervision and university teaching.
  • Maintain service records and documentation in line with NHS standards.

Qualifications and experience

  • Registered Pharmacist with the GPC.
  • Post‑graduate clinical pharmacy diploma or equivalent.
  • Post‑graduate Masters or doctoral level qualification relevant to renal medicine.
  • Experience in renal pharmacy practice.
  • Non‑medical prescribing qualification.
  • Teaching experience at undergraduate or postgraduate level (preferred).
