An excitingopportunity has become available for an enthusiastic and forward-thinking GeneralMedical Practitioner to join the First Coastal PCN in a new role that will bedeveloped as it progresses. Thepost-holder will manage a caseload and deal with a wide range of health needsin a primary care setting, ensuring the highest standards of care for allregistered and temporary patients.

At present,First Coastal PCN does not have Home Office sponsorship status. This means thatwe are currently unable to provide any assistance with visas and all applicants and employees must have indefinite right to work in the UK in order to qualifyfor the job roles we offer. This may change in the future due to ongoing work.

Main duties of the job

  • To undertakeduties governed by the contract and Direct and Local Enhanced Services including-
  • Avariety of duties including face to face surgery consultations, telephoneconsultations and queries, visiting patients at home, checking and signingrepeat prescriptions and dealing with queries, paperwork and correspondence ina timely fashion.
  • Ingeneral the post-holder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.

About us

First CoastalPCN covers a population of approximately 84,000 patients, this is made up ofseven practices, Marsico Medical Practice (Mablethorpe), Hawthorn MedicalPractice (Skegness), Beacon Medical Practice (Skegness), Merton Lodge (Alford),Spilsby Surgery, Stickney Surgery and Old Leake Medical Practice. Thesepractices work together to deliver care to the localpopulation.

Job responsibilities

ClinicalResponsibilities

  • To undertakeduties governed by the contract and Direct and Local Enhanced Services including-
  • Avariety of duties including face to face surgery consultations, telephoneconsultations and queries, visiting patients at home, checking and signingrepeat prescriptions and dealing with queries, paperwork and correspondence ina timely fashion
  • Making professional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation
  • Assessing the health care needs of patients with undifferentiated and undiagnosedproblems
  • Screening patients for disease risk factors and early signs of illness
  • In consultation with patients and in line with current practice disease managementprotocols, developing care plans for health
  • Providing counselling and health education
  • Admitting or discharging patients to and from the caseload and referring to other careproviders as appropriate
  • Recording clear and contemporaneous consultation notes to agreed standards
  • Collecting data for audit purposes
  • Administration – dealing with queries, patient test results, referral letters, task, emailsand other paperwork and correspondence in a timely manner
  • Compiling and issuing computer-generated acute and repeat prescriptions
  • Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
  • In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
  • Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondarycare or to other providers as appropriate
  • Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols as well as wider guidance such as that from NICE.

As this is a new and evolving role, this is not an exhaustive list of duties and responsibilities, and the post holdermay be required to undertake other duties that fallwithin the grade of the job, in discussion with their line manager. The content of this job description will be reviewed regularly in the light of changing service requirements and any such changes will be discussed with the post holder.

Person Specification

Experience

  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences -e.g. patients

  • Full driving license and use of car to travel around the PCN patch to deliver patient care.

Qualifications

  • Full GMC registration.
  • Completion of Certificate of Training (CCT) in GP
  • Performers List registered
  • The General Medical Practitioner must not be:
    • a. Prohibited from performing any such service by regulation 24 of the National Health Service England Regulations 2013 – performers Lists
    • b. Suspended from the list of medical practitioners maintained and published by NHS England in accordance with section 91 of the NHS Act 2006 or from the registers kept under section 2 of the Medical Act 1983;
    • c. and or Subject to interim suspension under 41A of the Medical Act 1983.
  • Experience of working in NHS General Practice.
  • Keeping up with CPD and good knowledge of current guidelines.
  • Understanding of QoF, Medicines Management and the general working of GP practices.
  • Understanding of SystmOne.
  • Excellent communicator and team player.
