Job overview
This is a great opportunity to join a supportive team providing rehabilitation under the guidance of clinicians to help people become more independent and reach their personalised goals after a stroke.
The role is based primarily in patients’ homes and local community and may involve providing personal care, meal preparation, and supporting care plans to improve mobility, dexterity, cognition, vision, hand and arm movement, or communication. The role may also include taking clinical observations.
Main duties of the job
- Carry out rehabilitation plans as prescribed by therapists
- Assist clinical staff in the assessment and treatment of patients
- Assist with activities of daily living as per care plan
- Participate in stock checking of equipment and basic administration tasks
- Attend meetings and training as needed
- Escalate concerns about therapy programmes or deteriorating patients to senior colleagues
- Work as part of a multidisciplinary team

Experience, Skills & Knowledge
- Experience of working with adults in a health and social care setting
- Able to work without direct supervision
- Able to work as part of a team
- Competent use of IT (emails, word documents, excel)
- Ability to prioritise tasks
- Experience of working in a community setting
- Experience of community rehabilitation or stroke rehabilitation
