This is an exciting opportunity to join the Outstanding’ East Riding Youth Justice Service in a newly created role as a Transitions Worker. The service received a maximum score in the HMIP Inspection in January 2026, with the report noting excellent work to achieve positive change for children and to keep children and the community safe. Assessing, planning and delivery were consistently strong across all areas. Work to keep children and communities safe was also exceptional, with practitioners recognising the factors that affected safety.

The position of a Youth Justice Practitioner Transitions worker will provide support for children aged 17+ within the Youth Justice Service to reduce reoffending, prevent crime and support with transitioning into adulthood.
Key responsibilities of the Transitions worker will include:
- Transition Planning: Develop and coordinate transition plans for young people approaching adulthood.
- Interagency collaboration: Work closely with partner agencies including Probation, Children’s Services, Adult Social Care, Housing, Education, Health, and community organisations to create holistic, person-centred support plans.
- Case Management: Work with children on both statutory and non-statutory Youth Justice disposals in the community and in custody.
- Risk Management: Support risk and vulnerability assessments and contribute to risk management plans in collaboration with case managers and partner agencies.
- Adherence to policies and legislation: Must work in accordance with relevant policies, guidance and legislation, including, the Code of Practice for Victims of Crime. This ensures their work is in line with statutory requirements and best practices.
