Overview

The post holder will have an understanding of the knowledge and skills of the professional activities within a Multidisciplinary Community Support Team within the field of Learning Disabilities. They will be responsible for managing a clinical caseload, and will liaise with both Social and Health care professionals to meet client need.

Responsibilities

DUTIES & RESPONSBILITIES:Contribute to the development, maintenance, and evaluation of systems and structures, which promote the rights, responsibilities and diversities of people with a learning disability through effective communication.The post holder will promote the health and well-being of clients by providing nursing assessment and advice to clients, carers and other professionals.

We would particularly welcome applications from Welsh speakers; but even if you don’t speak any Welsh, or want to develop your skills, don’t worry: the health board will support you with training.

Job responsibilities

Person Specification

Qualifications and/or Knowledge

  • Registered Nurse (Learning Disability) with current Nursing and Midwifery Council registration
  • Evidence of ongoing professional development relevant to nursing practice
  • Meet the academic and professional requirements for revalidation
  • Understanding of the health needs of people with learning disabilities, including associated health inequalities
  • Knowledge of person-centred care and the importance of therapeutic relationships
  • Awareness of safeguarding responsibilities for children and adults and how to escalate concerns
  • Understanding of risk assessment and clinical risk management within community settings
  • Knowledge of relevant legislation, including the Mental Capacity Act, Equality Act, and Human Rights-based approaches
  • Understanding of multidisciplinary and multi-agency working in community services
  • Knowledge of accurate record-keeping and digital documentation requirements
  • Practice Supervisor or Practice Assessor qualification (or willingness to undertake)
  • Additional training relevant to learning disability nursing (e.g., Positive Behavioural Support, sensory needs, physical health skills)
  • Training in behaviour support, mental health awareness, or communication approaches for people with complex needs
  • Understanding of community-based pathways and partnership working across health, social care and third-sector services
  • Awareness of trauma-informed practice and its relevance to people with learning disabilities
  • Awareness of public health initiatives relevant to people with learning disabilities

Experience

  • Contributing to the assessment, planning, delivery and evaluation of nursing care
  • Communicating complex, diverse or sensitive information to individuals, carers and professionals
  • Managing changing priorities in a clinical environment, including responding to urgent or unexpected situations
  • Work both autonomously and effectively within a multidisciplinary team
  • Supporting individuals with complex needs, including behaviours of concern, mental health needs, or dual diagnosis
  • Contributing to audit, quality improvement or service development activities

Skills and Attributes

  • Carry out holistic nursing assessments and deliver person-centred interventions
  • Prioritise, manage and organise a caseload in a community setting
  • Identify, assess and respond appropriately to clinical risk
  • Motivated to engage in reflection, supervision and continuous professional development
  • Some ability to speak, read and/or write Welsh, or an eagerness to learn
  • Support health promotion activities tailored to people with learning disabilities

Other

  • Works flexibly to meet the needs of the service, including occasional adjusted hours in response to clinical priorities
  • Travels independently across the locality to visit individuals in a range of community settings
  • Uses initiative to resolve day-to-day issues and seeks guidance appropriately for more complex matters
