About the role

We have an exciting opportunity to recruit a learning disability nurse to work as a Community Nurse in Complex Physical Health within Bromley. The post holder will work in the Bromley CLDT Complex Physical Health Learning Disability nursing team, focusing on the additional needs of people with complex physical health needs.

Key Responsibilities

  • Manage a caseload of adults, working autonomously with a broad spectrum of physical health issues.
  • Provide holistic, high calibre, evidence‑based, skilled nursing care, assessments, and interventions.
  • Facilitate a variety of health education and health promotion training and group work with clients and carers as needed.
  • Support students and junior staff as required and provide a rich learning experience.
  • Work collaboratively as part of a multi‑disciplinary team and in partnership with other agencies/services.
  • Lead on providing robust health liaison to acute and primary care to ensure service users can access mainstream health services.
  • Maintain awareness of issues surrounding diversity and multi‑cultural communities.
  • Apply evidence‑based nursing care to promote physical well‑being utilising LD specific and mainstream health targets.
  • Build skills in assessing, implementing, and critically evaluating intervention outcomes to maintain and improve the physical well‑being of service users.
  • Advise and work collaboratively with colleagues, carers, professionals and other agencies around complex clinical interventions and outcomes.
  • Implement NHS health care policies and strategies into nursing practice, including targets outlined in “Valuing People” and National Service frameworks.
  • Demonstrate existing relevant skills of working with adults with a learning disability and complex physical health needs through previous experience and apply these to the role.
  • Demonstrate a commitment to develop appropriate leadership skills as a nurse, acting as a role model for junior staff team members.
  • Contribute to the delivery of high‑quality evidence‑based nursing care.
  • Ensure accountability in practice, in line with the NMC code.
  • Employ health promotion techniques to develop overall health and well‑being of service users and prevent avoidable illness.
  • Liaise with multi‑disciplinary colleagues and other professionals/agencies to address issues that create barriers to social well‑being and report on unmet need and obstacles to senior colleagues.

Key Qualifications

  • Broad knowledge of the requirements of the NMC, the Trust and government policies and legislation and their application to practice.
  • Experience working with adults with learning disability and complex physical health needs.
  • Commitment to develop leadership skills in nursing and a capacity to act as a role model for junior staff.
  • Demonstrated accountability in practice, compliant with the NMC code.
