Your Position

• Assist patients after hospital discharge.
• Support clients in regaining independence and confidence.
• Deliver care in line with individualised care plans.
• Provide:
  • Medication administration
  • Monitoring vital signs
  • Exercise support and rehabilitation facilitation
  • Household chores
• Communicate with medical staff, including district nurses and pharmacists.
• Work independently and collaboratively as part of a care team.
• Maintain accurate electronic reporting and documentation.

Hours of Work

Full‑time contract up to 40 hours per week, with shift patterns ranging from 7:00 am to 8:30 pm. Flexibility to work different shifts is required to meet service needs.
