- To identify and coordinate the allocation of a beds for patients needing an inpatient admission as a result of a mental health crisis or planned acute admission and ensure safe and appropriate transfers.
- To monitor patient flow across all inpatient sites understanding the demand within the community and Crisis caseloads, enabling a streamlined approach to admissions and discharge.
- To be the main point of contact during some evenings and weekends to ensure that beds are appropriately managed and allocated as clinically indicated.
- Identify those patients that are placed on non-locality wards, Delayed Transfer of Care or Out of County and identify those that are pending discharge.
- To communicate and negotiate effectively highly complex information on availability and appropriateness of beds across the Trust and be able to ensure that all parties understand implications and consequences of decisions.
- The ability to utilise good personal skills and gain the agreement of relevant parties, particularly in relation to the admission of patients during periods of high demand.
- To keep accurate documentation of all bed management referrals and record on RIO in line with Trust policy.
- To repatriate outlying patients when appropriate and preferably to their locality ward and where this is not possible, monitor that they are being regularly reviewed and clinical detail updated.
- Maintain an accurate bed state of all acute inpatient beds and to monitor and keep records of peak periods.
