We have an exciting opportunity for clinical pharmacists to join our pharmacy team, working across community pharmacy and frailty services. This development role is suitable for newly qualified pharmacists or more experienced pharmacists who meet the higher-level criteria in the person specification.
Responsibilities
- Act as the responsible pharmacist in community pharmacies and deliver frailty services within CHCP.
- Participate in comprehensive geriatric assessment for patients in the Jean Bishop Integrated Care Centre, care homes, community bedded units, and the hospital at home frailty virtual ward (including home visits).
- Provide NHS services such as Pharmacy First, vaccinations, NMS and DMS within the community pharmacies.
- Collaborate with a multidisciplinary team that includes clinical pharmacists, doctors, specialist nurses, pharmacy technicians and therapists.
- CHCP has four established community pharmacies: Brocklehurst Chemists and City Health Pharmacy.
Qualifications & Requirements
- Qualification as a registered pharmacist; experience suitable for newly qualified or experienced pharmacists meeting higher-level criteria.
- Strong clinical skills and effective communication with patients, GPs and other healthcare professionals.
- Own reliable transport and willingness to work flexibly across the areas served by CHCP.
