An exciting opportunity has become available for an Advanced Practitioner in Social Work to work within the Prevent, Reduce and Enable Service, based within the East Adult Contact Team at Macclesfield Town Hall. The position is full time 37 hours a week, from Monday to Friday.
About the role
This position supports the Adult Social Care Teams in the East on the Prevent, Reduce and Enable Team, providing a quick and urgent response to individuals referred with a presenting need. It is a fast‑paced role working alongside a small team of Social Care staff, Occupational Therapy and the Reablement Service. The aim is to prevent further crisis or deterioration in needs and to prevent hospital admission. The Advanced Practitioner ensures practice standards are maintained; assessments are strengths‑based and person‑centred; interventions are proportionate to presenting needs; and staff receive guidance and supervision to tackle challenging conversations and conflict. The role involves a small caseload, safeguarding concerns and quality assurance for practice related issues.
About you
- Extensive knowledge of the Care Act 2014 and understanding of Cheshire East policies and processes
- A high degree of interpersonal and communication skills
- Ability to oversee effective teamwork and converse at ease with internal and external partners and customers
- Capacity to work flexibly and provide formal supervision to an identified group of staff
- Ability to oversee and manage safeguarding concerns and high‑risk situations
