Job Overview
We are seeking passionate and forward‑thinking Advanced Clinical Pharmacists to join our Clinical Streaming Hubs in the following areas:
- Porth Preseli (Withybush General Hospital)
- Cardigan Integrated Care Centre
- Eastgate (Llanelli)
We have 1 available post in each of the above 3 sites.
Responsibilities
- Provide expert clinical pharmacy support to the Clinical Streaming Hub.
- Provide comprehensive medication reviews for patients on the caseload (face‑to‑face, telephone or in a patient’s home) and prescribe/deprescribe appropriately within scope of practice.
- Work closely with multidisciplinary teams, including GP practices and community pharmacies, to ensure safe and effective medicines use.
- Support antimicrobial stewardship and medicines optimisation initiatives.
- Provide education and training on medications to other clinical staff across the Clinical Streaming Hub team, and supervise and mentor junior pharmacists, pharmacy technicians, and pharmacy students.
- Maintain accurate and thorough records of all patient interactions, using appropriate clinical templates and coding in line with service protocols.
- Lead on clinical audit of medicines prescribed in the service and take a proactive role in quality improvement initiatives, implementing recommendations where appropriate.
- Provide patients with clear, independent information regarding their medications and any changes to their prescriptions, referring to other healthcare professionals for additional support when necessary.
Qualifications
- Independent prescribing qualification is essential.
- Strong clinical and communication skills.
- Ability to work autonomously, prioritise workload effectively, and commit to ongoing professional development.
- Experience in developing specialist clinical expertise aligned to service priorities is an advantage.
