Job overview
Due to expansion and career progression within our team , we have an exciting opportunity for a Clinical Pharmacist to join our organisation .
We are looking for an enthusiastic and dynamic pharmacist to work within Nottingham City South PCN .
Completion of CPPE pathway and independent prescribing are desirable but not essential .
2 years experience of pharmacy work within the healthcare setting or equivalent is desirable but not essential .
Main duties of the job
The post holder will work as apart of a multi-disciplinary team in a partially patient facing role . The post holder will responsibility for areas of Chronic disease management , clinical medication reviews , supporting the general practice staff with regards to prescription and medication queries .
They will potentially provide support for the repeat prescription system, deal with accurate prescription requests, medicine reconciliation on transfer of care and system’s for safer prescribing , providing expertise in clinical medicine advise, while providing clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
