About the Role
We are looking for a full‑time Band 6 Mental Health Practitioner to join our Lincoln South CMHT. The post holder will work within a friendly Multi‑Disciplinary Team which includes Community Mental Health Nurses, Recovery Practitioners, Occupational Therapists, Nursing Associates, Community Support Workers, Psychiatrists, Psychologists and Admin staff. The role also involves working alongside Neighbourhood Teams, Integrated Placement Based Teams, Adult Social Care, GP surgeries and other partner organisations to ensure the best outcomes for our patients and carers.
Key Responsibilities
- Provide high quality person‑centred care to patients, families and carers.
- Act as care co‑ordinator, undertaking comprehensive evidence‑based assessments.
- Support service users to work towards recovery goals, build self‑resilience and promote independent living.
- Show enthusiasm for service development within a complex and changing service.
- Maintain flexibility, a ‘can do’ approach and the ability to work under pressure and adapt to changes.
- Travelling independently in the community to deliver services, supporting a hybrid approach to working.
Qualifications & Experience
- Experience as a Social Worker, Occupational Therapist or Mental Health Nurse.
- Passion and drive to deliver excellent patient care.
- Excellent communication and leadership skills.
- Enthusiasm for service development and willingness to participate in additional training and professional development.
