We are seeking an enthusiastic and skilled Band 5 Mental HealthPractitioner to join our friendly team at Crediton Hospital, supporting theCrediton and Okehampton community.

This is a fantastic opportunity for amotivated individual looking for a fixed term or secondment position to covermaternity leave until 31/10/2026. In this varied and rewarding role, you willhold a caseload of clients with severe mental illness (SMI), delivering evidence-basedinterventions and providing day-to-day therapeutic support.

You will workcollaboratively with an experienced multidisciplinary team, including band 6colleagues, the team manager, consultant psychiatrist, and psychologyprofessionals. The ability to travel across the area is essential to meet theneeds of our clients.

Main duties of the job

  • Hold and manage a caseload of clients with SMI, deliveringpersonalised, evidence-based interventions.
  • Carry out comprehensive risk and needs assessments,complete PROMS, and contribute to the development, review, and implementationof care plans.
  • Offer day-to-day therapeutic support and respond promptlyto crisis situations, ensuring accurate and timely clinical record-keeping.
  • Liaise closely with band 6 staff, the team manager,consultant psychiatrist, psychology, and the wider MDT to deliver holisticcare.
  • Conduct physical health monitoring and participate in dutycover as required.
  • Champion recovery, independence, and social inclusion forclients within the community.
  • Flexibility and the ability to travel throughout theCrediton and Okehampton area are essential for this role.

Person Specification

Knowledge

  • An understanding of the NSF and Mental Health Act/ Mental Health Capacity Act requirements.
  • An understanding of CPA Process
  • Understanding of recovery principles in Mental Health

Other

  • The ability to travel to meet the requirements of the role

Qualifications

  • An appropriate up to date professional mental health qualification eg RMN, OT or social worker on the appropriate professional body register and hold current registration with no restrictions. (It will be an essential requirement for newly qualified social workers to demonstrate that they have completed The Assessed and Supported Year in Employment (ASYE) before commencing in post or there will be a requirement to complete this within the first 12 months of employment with the Trust.)
  • Evidence of relevant continuing professional development

Experience

  • Postgraduate experience in at least two inpatient settings or substantial experience working within a mental health and/or learning disabilities setting;
  • Managing a caseload of clients with a range of mental health problems.
  • Working within a community setting in either the statutory or voluntary sector.
  • Demonstrates sound theoretical knowledge of mental health practice;
