Role Overview
The inpatient unit provides rehabilitation and end‑of‑life support for palliative patients and their families. Patients are referred from acute settings or directly through GPs and local community nursing teams. The team is led by a Matron and includes registered nurses, healthcare assistants, physiotherapists, occupational therapists, rehabilitation assistants, care management staff, a pharmacist and visiting medical officers.
Responsibilities
- Work collaboratively within a multidisciplinary team and independently to balance changing tasks.
- Use knowledge, skills and experience to make informed decisions and exercise initiative.
- Support multi‑agency collaboration when required.
- Safeguard adults and children, recognising risk of abuse, assessing capacity and assisting decision‑making.
- Maintain clear and accurate records of advice, observations and actions taken.
- Assess, plan, agree, implement and evaluate care, recognising limits and escalating concerns; respond to changes in a patient’s condition.
- Participate in audits, surveys and provide feedback on policy or procedural changes.
- Maintain professional registration and comply with professional code of conduct.
Qualifications / Requirements
Professional registration in the appropriate clinical role and compliance with the relevant code of conduct.
Experience working in inpatient rehabilitation or palliative care settings is desirable.
Ability to work within a team, exercise independent judgement and maintain accurate documentation.
Vaccination encouraged for clinical and non‑clinical roles that have direct or social contact with patients.
