Senior Clinical Pharmacist | Kent and Medway Mental Health NHS Trust

We are looking for an enthusiastic, motivated, and committed pharmacist to provide high‑quality clinical pharmacy service to the Medway Community Mental Health Together team (CMHT). Qualifications

• Independent prescribing qualification, or actively working towards it
• Post‑graduate qualification in pharmacy
• Experience in mental health pharmacy

Responsibilities

• Operational responsibility for the safe and effective delivery of medicines‑management services within the CMHT
• Facilitate pharmacy‑led clinics and medication reviews for patients requiring advice and guidance about prescribed medications
• Network and drive innovation in mental‑health medicines optimisation across the primary and secondary care interface, supporting patients closer to home
• Collaborate with GPs, primary‑care pharmacists, pharmacy technicians, the KMMH pharmacy team and secondary‑care clinicians
• Provide line management and area‑service support for pharmacy technicians working within the community mental health team
• Work with other pharmacists on audits and projects to evaluate and improve services, aligned with pharmacy and Trust priorities and objectives
