Clinical Lead / Registered Nurse (Domiciliary Care & Dementia)

Chiswick, West London

£45,000–£50,000 + bonus

CQC Outstanding service

Home Instead Chiswick is an Outstanding-rated domiciliary care service with a strong reputation, a well-established private client base, and a 16-person office team.

We’re now looking for a Clinical Lead / Registered Nurse to join our nurse-led leadership team, working alongside both our Registered Manager and Head of Care, who are experienced Nurses.

This Clinical Lead role is a community-based leadership position, designed for a Registered Nurse who wants to move beyond traditional settings into a more relational, person-centred model of care.

This role will suit:

  • A Registered Nurse with solid post-registration experience (ideally Band 6 level or equivalent)
  • Nurses with meaningful exposure to dementia and/or complex care
  • A Clinical Lead or aspiring Clinical Lead confident in clinical decision-making and risk management
  • Nurses who value relationship-led, person-centred care
  • A Registered Nurse looking to step into a more community-based, leadership-focused role

To be successful as a Clinical Lead / Registered Nurse, you will need:

  • Active NMC registration (RGN, RMN, or RNLD)
  • Strong UK-based experience as a Registered Nurse
  • Experience supporting people with dementia and/or complex care needs
  • Confidence working autonomously and supporting care teams and families
  • Ability to work in a community-based role across West London

The Clinical Lead role:

  • Provide clinical oversight (dementia and complex care)
  • Support and develop care professionals and office team
  • Act as a senior Registered Nurse within the service
  • Support families in more complex situations
  • Drive quality, governance, and best practice
  • Contribute to service development and growth

This is a hands on Clinical Lead role, split between our Chiswick office and client visits in the community.

We offer the opportunity for a Registered Nurse to step into a genuinely supportive, clinically led environment where quality of care, relationships, and professional development are prioritised.

If you are a Registered Nurse or Clinical Lead and this aligns with your experience, we’d welcome your application.
