Job Description:

Working alongside the existing Bed Manager role and Flow Lead, the post holder will provide senior clinical leadership for access to inpatient care within the acute service for adults of working age care group, ensuring that admission decisions support safe, timely and clinically appropriate care.

The post holder will be responsible for the effective clinical coordination of the patient pathway within bed management, ensuring safe purposeful admission as well as effective and timely transfer and discharge in line with clinical need, risk formulation and available capacity.

The post holder will support the existing Bed Manager in receiving and coordinating referrals for service users who have been gate‑kept for admission by the Crisis and Home Treatment Team (CHTT), ensuring that clinical risk, acuity and least restrictive options have been appropriately considered and that purposeful admission is clear.

Main duties of the job

Lead on the Case Management all OOA PICU and acute patients ensuring the patients care is clinically sound and supportive of the patient journey to step down/discharge in a timely effective and safe manner.

Act as the senior clinical facilitator to patient admission, reviewing the gatekeeping process and pathway, assuring each admission is purposeful. Working with crisis teams to consider the most appropriate course of action in relation to admission, alternatives and any other aspect of the patient admission pathway.

To actively participate in bed management/case review meetings held across Inpatient wards.

Provide clinical leadership to the bed management team members ensuring alongside the existing bed management that the team is providing safe effective and efficient service delivery, in line with the Trusts values, strategy and Efficiency planning.

The post holder will contribute to the existing governance meetings and arrangements. Make complex judgements on a daily basis regarding individual cases and be able to analyse and present strategically both the issues and solutions.

Clinical Leadership Responsibilities

The post holder will work with acute and community services to ensure that key data is recorded to enable evaluation/review of the pathway across different agencies so that areas of difficulty are speedily addressed.

The post-holder will be required to run reports as requested by Exec, Senior Managers and Inpatient services to show trends and analyse the demand and capacity for inpatient admissions.

The post holder will also be required to analyse the Trusts performance in relation to acute and PICU bed use in liaison with the Bed Management Team and benchmark DHcFT with other organisations using relevant measures.

Policy/Service Development

Develop networks with staff both internally and externally across DHcFT to ensure a consistent approach across Derbyshire.

To raise awareness of the changing need for acute in-patient beds, so that hospital beds are used more efficiently, resulting in a reduction in hospital admissions/stays. Support the bed reduction plan.

To review the systems which are in place to monitor and ensure the effective use of acute and PICU beds

Provide leadership for the ward teams in developing systems and processes for effective admission and discharge care planning

Support the delivery of cost reduction programmes

Provide cover of other specialties within the bed management team and deputise for the Flow lead as necessary.

Coordinate the resolution of complaints, conflicts and issues from patients, staff, in a timely manner in line with Trust policies.

To work with clinical and governance colleagues to undertake root cause analyses and trend analyses of complaints and serious incidents.

The post holder will work as part of the flow team to optimise inpatient capacity, maintain safe patient flow and support effective clinical decision-making across acute care pathways.

The post holder will provide clinical leadership to the Bed Management Team and work closely with Divisional Operations Managers, Matrons, medical, nursing and multidisciplinary teams to ensure that decisions relating to admission, transfer and discharge are clinically robust, person-centred and risk informed.

Qualifications

  • Registered with Professional Body (NMC/HCPC ) Professional Clinical Qualification; RMN
  • Evidence of Continuing Professional Development

Skills and experience

  • Ability to influence and negotiate at a senior level within the division and wider trust.
  • Experience of clinical leadership in the NHS
  • Experience of managing and deciding on complex clinical issues
  • Experience of working with stakeholders across teams and evidence of developing positive productive working relationships
  • Ability to work effectively as part of a team and an ability to prioritise workload and deliver utilising the support of appropriate staff.
