Discharge & Care Coordination Social Worker (Hybrid)

Sanctuary Personnel is seeking a Social Worker for the Transfer of Care Hub Team in Warrington. This ongoing, full-time role offers £30.00 per hour with around 37 working hours per week, utilizing a flexible hybrid model of around 3 days onsite.

In this role, you’ll coordinate safe hospital discharges, conduct assessments, and collaborate with multi-agency teams. Ideal candidates will have a relevant degree, registration with Social Work England, and recent experience in hospital settings.
