Job Title: Social Worker – Adult Hospital Discharge
Location: Halton
Rate: £35 per hour
Liquid Personnel is currently recruiting for a Social Worker to join a Hospital Discharge team based in Halton.
Responsibilities
In this role, you will play a key part in facilitating safe and timely hospital discharges, especially during the challenging winter months. You will manage a complex caseload independently and efficiently, ensuring that each case receives the attention it deserves. Your support will be crucial for individuals and families transitioning from hospital to community care, helping them navigate this critical period with ease. Collaboration with multidisciplinary teams will be essential to ensure seamless, person-centred care, making sure that every individual’s needs are met comprehensively. This role requires a proactive and compassionate approach, dedicated to improving the lives of those you serve.

Qualifications and Experiences
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
