Job Description
As a Social Worker within our Carers Team, you’ll play an essential role in supporting unpaid carers and the people they care for. You’ll manage your own diverse caseload, applying person‑centred, strengths‑based practice to help individuals make informed decisions and achieve meaningful outcomes. Your work will span Care Act assessments, hospital discharge planning, safeguarding enquiries, Mental Capacity Act assessments, and the development of tailored care and support plans. You will work confidently within legal and policy frameworks—including the Care Act, Mental Capacity Act, Mental Health Act, Human Rights and Equalities legislation—ensuring every intervention is lawful, proportionate and focused on maximising independence and reducing risk.
In this role, you’ll build effective relationships with service users, carers, families and partner organisations, working collaboratively to deliver safe, timely and creative solutions. You will respond to referrals, assess levels of urgency, and take appropriate action, including in emergency situations to protect vulnerable adults or children. You’ll maintain accurate case notes, prepare professional reports (including court documentation where required), and contribute to multidisciplinary decision making around Continuing Health Care, Section 117 aftercare and Human Rights assessments. As part of a supportive and reflective team, you’ll actively participate in supervision, performance development and service improvement activities, ensuring your practice remains informed, current and aligned with our commitment to high quality social work.
