What you will do

  • Early Intervention: Support adults through preventative and strengths-based interventions designed to reduce escalation of needs.
  • Assessment: Undertake assessments and develop person-centred support and care plans.
  • Care Planning: Create effective plans that promote independence, resilience, and positive outcomes.
  • Multi-Agency Working: Collaborate closely with occupational therapists, mental health services, police, and voluntary organisations.
  • Case Management: Manage a varied caseload and ensure timely interventions are delivered.
  • Partnership Working: Liaise with external agencies and professionals to coordinate holistic support.
  • Record Keeping: Maintain accurate case records, reports, and documentation in line with organisational requirements.
  • Advocacy: Promote the rights, wishes, and wellbeing of service users throughout the support process.

Qualifications / Requirements

  • Social Work Qualification: Degree in Social Work or equivalent recognised qualification.
  • Registration: Current Social Work England registration.
  • Experience: Previous experience within Adult Social Care services.
  • Assessment Skills: Ability to undertake strengths-based assessments and develop effective support plans.
  • Multi-Agency Experience: Experience working collaboratively with partner agencies and multidisciplinary teams.
  • Communication Skills: Excellent interpersonal, organisational, and report-writing abilities.
  • Decision-Making Skills: Ability to manage competing priorities and make informed professional decisions.
