What you will do

  • Prepare for and engage with individuals, families, carers, groups, and communities to assess their needs and circumstances
  • Support individuals in making informed decisions and manage situations to achieve positive outcomes
  • Develop, implement, and evaluate plans that foster growth, development, and independence for older persons
  • Respond effectively to crisis situations and advocate on behalf of individuals and groups
  • Manage risks, ensuring the safety of individuals, families, carers, and yourself
  • Work within multidisciplinary teams to promote best practices in social work
  • Demonstrate professional competencies by maintaining your own development and contributing to the team’s success
  • Ensure integration of service provision through regular liaison with related organisations

Qualifications and Requirements

  • Registration as a Social Worker with Social Care Wales
  • Diploma/Degree in Social Work or a predecessor qualification
  • Completion of the Consolidation of Practice qualification within the first three years of practice if qualified after

Experience

  • Experience in assessment, care planning, and service delivery monitoring
  • Experience working within a community setting
