What you will do
- Prepare for and engage with individuals, families, carers, groups, and communities to assess their needs and circumstances
- Support individuals in making informed decisions and manage situations to achieve positive outcomes
- Develop, implement, and evaluate plans that foster growth, development, and independence for older persons
- Respond effectively to crisis situations and advocate on behalf of individuals and groups
- Manage risks, ensuring the safety of individuals, families, carers, and yourself
- Work within multidisciplinary teams to promote best practices in social work
- Demonstrate professional competencies by maintaining your own development and contributing to the team’s success
- Ensure integration of service provision through regular liaison with related organisations
Qualifications and Requirements
- Registration as a Social Worker with Social Care Wales
- Diploma/Degree in Social Work or a predecessor qualification
- Completion of the Consolidation of Practice qualification within the first three years of practice if qualified after
Experience
- Experience in assessment, care planning, and service delivery monitoring
- Experience working within a community setting
