Social Work Support Assistant – Adult Social Care
Permanent, full‑time (37 hours) countywide Learning Disabilities Team.

About the role
As a key worker for adults over 18 with care needs, you will be the main point of contact, provide support and advice, and help them live as independently as possible.
You will complete needs assessments, consider safeguarding, review healthcare needs and promote self‑directed support.
You will work within the North Locality Team, collaborating with social workers, care managers, GP practices, community mental and physical health services, and district nurses.
Key focus is managing risk and protecting service users from serious harm.
Strong team and independent working, time management, prioritisation, interpersonal, advocacy and communication skills are essential. Solid IT skills are required.
Responsibilities
- Carry out assessments and collaborate with service users to design solutions, seeking guidance from managers and colleagues.
- Plan and manage work to allow monitoring and review of outcomes.
- Undertake carers’ and risk assessments as part of the care management process.
- Participate in the duty system to pick up ad hoc work and act on information under supervision.
- Shadow students and newly appointed staff from other disciplines.
- Participate in discharge planning (from hospital) and care planning meetings.
- Maintain written and electronic records to organisational standards.
- Provide reports, outcomes of assessments and recommendations.
- Advocate on behalf of service users.
Essential requirements
- NVQ Level 3 in social care or equivalent.
- Experience working with adults in a social care setting.
- Knowledge of legislation relevant to mental health and adult social care.
- Experience working with people experiencing mental illness, learning disability and/or physical conditions in adults and older people.
- Valid driving licence and access to a car for work purposes.
