Job Overview

Social Worker – Integrated Services for adults with disabilities at the Millennium Centre, Leasowe, Wirral.

Responsibilities

  • Conduct Care Act assessments.
  • Develop personalized support plans.
  • Complete mental capacity assessments.
  • Lead safeguarding enquiries.
  • Support adults with disabilities and their carers to achieve desired outcomes.
  • Apply asset‑ and strength‑based approaches to assessment and support planning.

Qualifications

  • Social Work qualification – Degree in Social Work, DipSW or equivalent.
  • Registered with Social Work England (SWE).
  • Evidence of continuing professional development in line with the Professional Capability Framework.
  • Completion of the Practice Educator Award.

Knowledge and Expertise

  • Practise in line with the core principles of the College of Social Work and the Families and Well‑being Directorate.
  • Knowledge of relevant legislation – Adults: Mental Health Act 1983, Mental Capacity Act 2005, Disability Discrimination Act 1995, Care Act 2014, Safeguarding regulations.
  • Understanding of confidentiality and data protection requirements.
  • Effective verbal and written communication with families, professionals and other stakeholders.
  • Awareness of current practice issues and challenges facing social workers.
  • Commitment to anti‑discriminatory practice.
  • Ability to work with individuals, families and carers from diverse backgrounds.
  • Willingness to work across teams as required by the service.
  • Excellent customer service skills and partnership working with council and partner agencies.
  • Sound assessment and interviewing skills.
  • Proficiency with IT – e.g., Microsoft Office.
  • Emotional intelligence and resilience relevant to the role.

Experience

  • Proven experience working with carers.
  • Experience working with vulnerable individuals and families.
  • Experience arranging support plans and/or packages of care.
  • Experience working with partner agencies professionally.
  • Experience in a Local Authority setting.
  • Experience working in a multi‑agency team.
  • Experience with a range of families with diverse issues and needs.
