West Leeds Primary Care Network is looking for an organised and passionate Care Co-ordinator/Nurse.
In this role, you will be responsible for supporting the delivery of the Proactive Care Contract, engaging with patients and coordinating their access to services. You will also deliver travel vaccine clinics across six practices.
The position offers a salary of £32,000 to £36,000 depending on experience.
