What is the job role?
We have exciting Community Case Worker posts within our One Coventry Integrated Team. You will be supporting people who have a wide range of social care issues, reducing hospital admissions, improving the flow out of the hospital, and promoting independence. This is a great opportunity for motivated individuals who enjoy delivering person‑centred support.
We have two permanent full‑time positions available.
Who are we looking for?
We are looking for a person with a passion for promoting independence and making the principles of the Care Act 2014 a reality for the people of Coventry. If you like to work as part of a team but can also use initiative and work autonomously, we would really like to hear from you.
- Experience of assessment and case management work with older people, people with physical disability, people with learning disability or other adult service users.
- Excellent communication skills.
- Ideally have experience of recording and record‑keeping procedures.
- Experience of team membership and participation.
- Experience of working with a range of service user groups either in a personal or professional capacity.
- Very good organisational skills.
This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).
