Responsibilities

• Conduct comprehensive assessments and devise care plans for recently discharged patients.
• Coordinate with healthcare professionals to ensure continuity of care.
• Attend daily meetings at the hospital to review patient progress.
• Offer support and guidance to patients’ families and carers.
• Document and report on patient progress and any areas of concern.
