Job Description:

We have an exciting opportunity for an independent, autonomous Prescribing Clinician to play a key role in our Care Coordination and Integrated Urgent Care services across the Bath, Swindon and Wiltshire locality on a permanent basis. We can support both part‑time and full‑time working arrangements on a fixed rota pattern basis. This role offers a unique blend of clinical assessment and system navigation, supporting patients to access the right care, in the right place, at the right time, helping to reduce unnecessary hospital admissions and improve patient outcomes.

You will work across our Care Coordination service during regular hours and support our Urgent Care service out of hours. Care Coordination shifts are based at the Chippenham Head Office, while out‑of‑hours sessions will be based at one of our Primary Care Centres.

Role

  • Act as a senior clinical decision‑maker, supporting the triage, assessment and redirection of patients across the wider health and care system.
  • Work collaboratively with ambulance services, primary care, community services and secondary care to ensure patients are safely managed in the most appropriate setting.
  • Provide real‑time clinical support to ambulance crews, identifying suitable alternative pathways to avoid unnecessary conveyance to Emergency Departments.
  • Deliver clinical assessment, diagnosis and prescribing, where appropriate, through a combination of remote consultations, face‑to‑face care and home visits.

Key Responsibilities

  • Deliver safe, effective clinical triage and prioritisation, managing undifferentiated presentations across all age groups.
  • Support system‑wide patient flow, ensuring timely access to appropriate services and reducing pressure on urgent and emergency care pathways.
  • Provide clinical advice and decision support to colleagues and partner services, including ambulance teams.
  • Undertake remote consultations and care planning, facilitating onward referral where required.
  • Contribute to the development of an integrated “whole system” approach to care delivery.
  • Support the ongoing improvement of care coordination pathways and clinical effectiveness.

About You

You will be a registered nurse, paramedic or allied health professional with a non‑medical prescribing qualification, able to work confidently within a broad scope of practice.

Skills & Experience

  • Experience working within urgent care, primary care or emergency care pathways.
  • Strong clinical reasoning and triage skills, with the ability to manage risk and uncertainty.
  • Ability to work autonomously and make complex decisions under pressure.
  • Understanding of system pressures and patient flow within urgent and emergency care.
  • Experience supporting or working alongside ambulance services or care coordination functions (desirable).
  • MSc in Advanced Practice (achieved or working towards), or equivalent experience.
  • Minimum of 2 years prescribing experience.
  • Completion of a recognised clinical reasoning module (e.g. PADRIP or equivalent).
