Please note the salary advertised is for a full‑time post so will be pro‑rated accordingly, as this is a part‑time post.

At Peterborough City Council’s Transfer of Care Team, we believe in the power of change and the importance of community. We are looking for enthusiastic social workers to join our friendly, fun, and cake‑loving team! Our team provides essential support for adult service users, ensuring safe and timely discharges from hospital. Working closely with Peterborough City Hospitals multi‑disciplinary teams, you will enable discharges within the Discharge to Assess framework.

Responsibilities

  • Provide essential support for adult service users, ensuring safe and timely discharges from hospital.
  • Work closely with Peterborough City Hospitals multi‑disciplinary teams to enable discharges within the Discharge to Assess framework.

Qualifications

  • Current registration with Social Work England.
  • Completed the AYSE.
  • In‑depth knowledge of current legislation and guidance, as well as national policy and practice developments.
  • Full driver’s licence and ability to travel between different locations.
  • Willingness to learn new things and maintain own continuing professional development.
