Overview

Community Social Worker – Community Mental Health Team (South) ForgeThe Community Mental Health Team based at The Forge (Port Talbot) is looking for a full‑time Social Worker with community mental health and ideally AMPH experience, along with a rounded knowledge of current best practice in mental health.

The CMHT in the borough is a well‑established multi‑disciplinary team, comprising mental health practitioners from both Neath Port Talbot Council and Swansea Bay University Health Board. The CMHT works closely with primary care, Police, Health Board, Drugs and Alcohol Services, Crisis Resolution and Home Treatment team, and Single Point of Access to develop excellent partnership arrangements.

Responsibilities

You will be expected to work within the legislative framework of the Mental Health Act 1983 (amended 2007) and the Mental Health Measure (Wales) 2010, and to demonstrate an understanding of how to work in a strength‑based, outcome‑focused way.

Social Workers in the team undertake assessments, reviews and care planning, and need to have the enthusiasm to play an active role in delivering a high‑quality service to our service users.

Qualifications

Social work qualified and professionally registered as a social worker with Social Care Wales.

Substantial post‑qualifying experience, with at least 6 months experience working within a mental health service.

Experience of working as an Approved Mental Health Professional is desirable.

Knowledge and working practice of the Social Services and Wellbeing (Wales) Act 2014, outcome‑focused assessment and a working knowledge of the Mental Health Measures (2010), Continuing Health Care, the Mental Capacity Act 2005 including Deprivation of Liberty Safeguards.
