NHS seeks a Community Nurse to join the Out of Hours Teams based at Norwich and St James’ Kings Lynn, providing urgent response care in homes across Norfolk and Waveney.
The ideal candidate will have at least 2 years community experience and relevant qualifications. Responsibilities include assessing patient care needs to prevent hospital admissions and working closely with multidisciplinary teams.
This position involves a 7-day service with night and weekend shifts.
