Description
Salary £36,363 – £40,777 per year. Full‑time, 37 hours per week. Urgent Care and Short Term Support. Location: Accrington, East Lancashire.

Responsibilities
- Conduct assessments of individuals in crisis or who are acutely unwell in community and hospital settings.
- Develop and implement care plans to promote independence and well‑being.
- Provide accurate care assessments and liaise with NHS colleagues and other professionals.
- Work within a multidisciplinary team to deliver services on a 7‑day schedule, including up to one weekend per four.
- Travel to various locations as required, using personal vehicle; a valid driving licence is essential.
Qualifications & requirements
- Registered Social Worker (SWE) or applicant to register with the Social Care Council.
- Experience in Intermediate Care settings and hospitals desirable.
- Strong communication, advocacy and relationship‑building skills.
- Ability to manage fluctuating workload and prioritise patient needs.
- Flexible schedule and willingness to work weekends and bank holidays.
- Valid driving licence and access to own vehicle; alternative arrangements considered for disability.
