Social Worker, Rapid Response & Reablement Team

Sanctuary Personnel, an innovative and committed recruitment agency, has a new permanent position available for a Social Worker within the Rapid Response & Reablement Team to work Full Time based in Wiltshire.

The salary for this permanent Social Worker role is up to £75,789 per annum.

  • To promote and deliver a high standard of care based on best practice to maintain patient/service user safety and provide an excellent standard of care.
  • To coordinate the assessment of patients/service users with complex needs including individuals with unconfirmed diagnoses.
  • To lead a team of staff and provide specialist advice in acute and chronic situations ensuring that the patients/service users needs are met along with their support network.
  • To ensure patient/service user involvement in the planning, delivery and evaluation of care, respecting their beliefs and dignity.
  • To be accountable for ensuring that safeguards are in place for patients/service users and to escalade any safety, safeguarding or risk concerns following the appropriate processes.
  • To build and advocate a therapeutic relationship with patients/service users and their support network, communicating with them in a way that respects their views, language, cultural differences and cognitive function.
  • To manage conflict between individuals including staff, patients/service users and their support networks.
  • To establish and promote effective working relations with all members of the multi-disciplinary team and outside agencies, clearly relaying condition related information in a timely and effective manner.
  • To provide clear, concise, and accurate electronic and paper records of the care given in accordance with agreed policy and standards.
  • To lead and coordinate nurse-led and multi-agency activities within the area of specialist practice.
  • To lead in the recruitment of team members and ensure a positive and supportive working environment for all staff.
  • To organise own workload and that of others, demonstrating the ability to recognise changing priorities and adjusting plans accordingly.
  • To lead shift planning/work scheduling for the service area including the appropriate allocation of staff to ensure safe levels of care.
  • To lead and contribute to service developments and propose changes to policies and procedures, ensuring that new initiatives are incorporated into working practices of staff.
  • To lead, coordinate and delegate activities to colleagues where applicable within agreed systems of working, considering required skill mix and staffing levels, in accordance with patient/service user need.

Requirements

  • Degree or Equivalent in Social Work.
  • Current Social Work England Registration.
  • Post Qualifying Social Work Experience.
