Part-Time Hours: 37.5 hours per week

Full‑time hours at Change Grow Live are 37.5 hours per week. For part‑time roles, salary and payments will be pro‑rata based on contracted hours.

About the role

With this role you’ll be working as part of the hospital inreach team based at both Liverpool and Aintree Hospitals.

The post holder will work across both hospital sites (e.g. A&E and admissions wards) to provide rapid access to assessment, brief interventions, harm minimisation and support discharge planning/onward referrals for people attending hospital with substance misuse related issues.

  • Working closely with Recovery Coordinators and partner agencies to develop pathways to care
  • Support service users in opiate and alcohol interventions within a hospital setting.
  • Support harm reduction interventions along with rapid access to treatment assessments.
  • Keeping the people who use our services at the heart of everything you do.
  • Facilitate training with hospital professionals.

About you

  • Good interpersonal skills and a willingness to work flexibly as part of a team
  • A clear understanding of the need for and ability to deliver quality services
  • Experience of working within substance misuse or a social care setting is essential
  • Strong understanding of substance misuse issues and experience of working within a similar field
  • Experience of working with groups/pods and managing group dynamics, such as conflict or challenging behaviour
  • Excellent verbal and written communication skills with good IT skills
  • Proactive and able to work on own initiative with excellent time management and prioritising skills
  • Good working knowledge of mental health interventions, services and good practice
  • Supportive team player with strong interpersonal skills, able to work in partnership with a wide range of professionals, agencies and stakeholders
  • Ability to manage change successfully while prioritising the needs of service users
  • Understanding of information governance processes and commitment to apply all necessary safeguards
  • Seek out learning opportunities to improve and broaden your professional knowledge and skills and contribute to learning and development of others
  • Good organisational skills
  • Confidence to work on own initiative as well as part of a team
  • Communicate confidently and effectively, verbally and in writing
  • Respond flexibly to the demands of the post
