Social Worker – Integrated Care & Reablement (6‑Month)

Liquidpersonnel is looking for a dedicated and motivated Social Worker to join their Intermediate Care Service in Southwark, London Borough. This 6-month fixed-term role, focuses on supporting residents, addressing care needs, and ensuring high-quality support through a strengths-based approach.

You will work with multi-disciplinary teams, carrying out assessments and managing safe, effective care plans. Ideal candidates should be registered Social Workers passionate about promoting independence, with strong knowledge of social care legislation and excellent communication skills.
