Overview
We have an exciting opportunity for clinical pharmacists to join our pharmacy team, working across community pharmacy and frailty services. This development role would suit either newly qualified pharmacists or more experienced pharmacists who meet the higher-level criteria in the person specification.
Responsibilities
- Split the working week between acting as the responsible pharmacist in our community pharmacies and delivering frailty services within CHCP.
- Provide comprehensive geriatric assessment to patients in the Jean Bishop Integrated Care Centre, care homes, community bedded units, and the hospital at home frailty virtual ward (including home visits).
- Communicate effectively with customers, patients, GPs and other healthcare professionals.
- Deliver NHS services such as Pharmacy First, vaccinations, NMS and DMS.
- Participate in professional support and training through an individualised induction programme.
Qualifications
- Pharmacy degree (or equivalent) and registration as a pharmacist with the General Pharmaceutical Council.
- Either newly qualified pharmacist or a more experienced pharmacist meeting higher-level criteria outlined in the person specification.
- Strong clinical skills and ability to work as part of a multidisciplinary team.
- Excellent communication and customer service skills.
- Own transport and ability to travel across the areas where services operate.
- Flexibility to work as required to meet organisational needs.
