Job Description

Liquid Personnel is recruiting a Social Worker for its client’s Mental Health Hospital Discharge Team at Stockport.

Responsibilities

  • Part of an all‑age adult mental health social work team, focusing on completing proportionate care act assessments and support planning to enable people to be promptly discharged from hospital when detained under s3 of the Mental Health Act.
  • Collaborate with Pennine Care colleagues and play a crucial part in improving mental health services and outcomes for people leaving hospital and their carers.
  • Support older and working‑age adults with functional or organic mental health diagnoses to be discharged from hospital safely and timely.

Qualifications & Experience

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
  • Post‑qualified experience
  • Degree level or equivalent in Social Work
