Job Description:

Our client, based in Telford is seeking an experienced Adult Social Worker to join their Early Intervention & Prevention Team.This role sits at the front door of Adult Social Care, focusing on early support, prevention, and reducing long-term dependency on services. You will work in a strengths-based, community-led way to empower individuals to maintain independence and achieve positive outcomes within their own homes and communities. This is an excellent opportunity for a confident practitioner who is passionate about asset-based practice, proactive intervention, and managing complex cases, while balancing risk and promoting independence.

What will your responsibilities be?

  • Undertake Care Act 2014 assessments, reviews, and support planning using a strengths-based approach
  • Deliver early intervention to reduce, delay, or prevent long-term care needs
  • Manage a diverse caseload including adults with complex needs
  • Complete Mental Capacity Assessments (MCA) and contribute to Best Interest decisions
  • Lead and support safeguarding activity including Section 42 enquiries
  • Develop person-centred, asset-based care and support plans
  • Promote “home first” principles to maximise independence
  • Work collaboratively with multi-agency partners including health and voluntary sector
  • Participate in peer reviews and reflective practice
  • Undertake duty responsibilities including responding to urgent risks
  • Maintain accurate case records and reports including Court of Protection documentation
  • Assess and manage risk using evidence-based decision making

Qualifications and Experience:

  • Qualified Social Worker (MA, BA, BSc, Diploma in Social Work or CQSW)
  • Registered with Social Work England
  • Minimum 2 years post-qualified experience including ASYE
  • Experience in Adult Social Care including assessment, care planning and safeguarding
  • Experience of strengths-based and preventative approaches
  • Experience managing complex cases within community settings
  • Strong knowledge of Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures
  • Experience contributing to Best Interest decisions and Court of Protection work

  • Strong assessment and analytical skills
  • Ability to manage a demanding caseload independently
  • Excellent communication and interpersonal skills
  • High standard of report writing and recording
  • Ability to apply professional curiosity and reflective practice
