We are looking for a Social Worker Assistant to support vulnerable adults in a specialist mental health and Acquired Brain Injury rehabilitation setting. You will play a key role in helping service users achieve greater independence, wellbeing and successful community outcomes by working as part of a multidisciplinary team.
Responsibilities
- Assist a Registered Social Worker in developing and supporting service users’ plans to achieve greater independence and community success.
- Engage with service users and their families or significant others to gather information and provide support around the assessment process.
- Liaise with external agencies to facilitate the implementation of plans that support service users identified needs and goals.
- Maintain detailed and accurate records of all activities and interactions.
