The Accommodation Hub is a newly established, substantive team within adult social care, evolving from a well-regarded care home team formerly known as the Peripatetic Team. This is an exciting opportunity to join us at a key stage of development and play a real role in shaping the future of adult social care in Kirklees.

About the Role

As an Experienced Social Worker in the Accommodation Hub, you will:

  • Work with adults living in care home settings and extra care housing
  • Hold a diverse caseload and carry out holistic assessments and reviews under the Care Act 2014
  • Participate in the team duty rota, responding to urgent needs and supporting colleagues
  • Champion a strength-based, person-centred approach, ensuring the voice of the individual, their carers, and representatives is central to all decisions
  • Support adults with a range of needs, including older people and individuals with physical disabilities, learning disabilities, and mental health needs
  • Take a proactive role in identifying and managing safeguarding concerns, working in partnership with others to keep people safe while promoting independence and choice
