Rehabilitation Support Worker

We are looking for a Band 3 Rehabilitation Support Worker to join our expanding and award‑winning services, which provide healthcare at home, prevent admission to our hospital and support discharge home.

Responsibilities

• Support senior clinicians in the assessment, planning, implementation and review of individual rehabilitation and care programmes for clients.
• Independently implement these rehabilitation and care programmes as appropriate.
• Teach prescribed exercise programmes, practise transfers and mobility, assist and promote independence in personal care and domestic tasks, deliver small items of equipment and document interventions accurately in notes in line with Trust guidelines.
• Treat clients, often unsupervised, according to agreed care plans and protocols.
• Report back to the relevant professional or team leader as required, regarding any changes in the client’s condition.
• Seek timely advice and support as appropriate.
• Work as part of a flexible multidisciplinary team and undertake tasks delegated by Occupational Therapists, Physiotherapists, Nurses, Social Workers and/or team leaders in a variety of settings primarily in the client’s home or community.
• Communicate effectively with clients and their carers, particularly clients with impairments which may affect their ability to communicate their needs and/or give informed consent to treatment.
