Adults Social Worker: Empower Outcomes & Independence

Portsmouth City Council is looking for a qualified Social Worker to join the Adults Intermediate Care Team. You will be responsible for assessing the needs of adults and their carers, and collaborate with others to implement creative solutions under the Care Act 2014.

This role involves using outcome-led interventions to support individuals, completing assessments, and managing your own caseload. Ideal candidates will have a social work qualification, knowledge of safeguarding, and effective communication skills.
