Your role
The role is a deeply rewarding position within the team that supports residents of the borough back into their own homes. As a Reablement Officer you will be part of the team providing integrated, multidisciplinary programmes of Reablement across the borough. The Reablement service operates 7 days a week, 365 days a year, from 07:00 to 22:00 and may exceed normal hours in line with service needs. You will work one weekend in four/five and your hours will be 09:30 to 17:30.
About you
You must be creative and self‑motivating with a strong customer care ethic. You will be able to demonstrate good interpersonal and communication skills and the value of promoting independence. You will be the first face‑to‑face contact from the Reablement service in the person’s home, using an asset‑based approach to promote independence, helping people to remain in their homes and connect within their communities to enhance their well‑being. You must hold a current valid driving licence and be able to travel as required. You will support the existing team in taking referrals and will be responsible for assessing, monitoring and reviewing Reablement programmes, including equipment and adaptations. You should have experience working with people who may require support with medication and moving and handling.
You will need to hold
- Adult Lead Level 3 in Health & Social Care or commitment to obtain within the first 6 months, and willingness to obtain the Care Certificate.
- Recognised certificate in Trusted Assessor or willingness to obtain.
- Medication Level 2 certificate.
- Enhanced DBS.
