We are seeking a dynamic, clinically credible practitioner with a passion for falls prevention, harm reduction and improving patient safety outcomes.

You will work both autonomously and as part of a small specialist team to lead and support the delivery of high quality, evidence based falls prevention practice across the Trust. The role combines clinical expertise, practice development, education, quality improvement and multidisciplinary collaboration to reduce falls related harm and improve patient outcomes.

The role will focus on the early identification of individuals at risk of falls and fractures, and the implementation of evidence-based prevention and risk reduction strategies. Working as an autonomous practitioner with advanced specialist knowledge and skills in falls prevention and management, the post holder will be accountable for the delivery of high quality clinical care, and for providing expert advice, education and support to multidisciplinary teams across the organisation.

A core component of the role will be the design, delivery and evaluation of education and training programmes to enhance staff knowledge, competence and confidence in falls risk assessment, prevention and post fall management.
