We are pleased to announce a unique and highly rewarding opportunity for a Specialty doctor to join the Hospital at Home (HAH) and Same Day Emergency Care (SDEC) services at Darent Valley Hospital. This newly created post is part of a pioneering cross‑sector expansion initiative designed to enhance our established care pathways. By joining our team, you will contribute to shaping the future of innovative healthcare delivery.
Role Context
The HAH, SDEC, and Frailty Assessment Unit are strategically integrated within the SDEC Village, facilitating collaboration across multiple specialties. In this role, you will work closely with our emergency department and the medical and surgical directorates to optimise both step‑up and step‑down care for our patients. As a member of the HAH service, which reports directly to the executive team, you will have the opportunity to impact patient outcomes while helping to lead service enhancements.
Key Responsibilities
- Support and enhance the Hospital at Home and SDEC services.
- Triage referrals, review patients and management plans, working seamlessly between SDEC and HAH.
- Collaborate closely with the clinical lead and lead nurses in Hospital at Home, contributing to the development of new patient pathways.
- Explore potential service developments and support multidisciplinary team work.
- Be involved in the development of medical students, trainees and allied health professionals.
