Overview

Tonbridge PCN is seeking to recruit an Advanced Nurse Clinical Practitioner (ACP) to support urgent care delivery across our PCN practices. The role involves triage, managing on‑day primary care, and communicating with GP and MDT colleagues for the ongoing follow‑up and treatment of patients.

Applicants will need to work across multiple sites, including evenings and Saturdays.

Responsibilities

• Triaging patients and providing acute primary care on the day.
• Communicating effectively with GP and MDT colleagues to coordinate ongoing care.
• Managing patient follow‑up and treatment as required.
• Working across multiple practice sites, including evenings and Saturdays.
