What you will do
- Caseload Management: Manage a caseload of young people transitioning into adult services.
- Assessment & Planning: Complete Care Act assessments, reviews, and person-centred support plans.
- Mental Capacity: Conduct Mental Capacity Assessments (MCA) and Best Interest Decisions (BID).
- Safeguarding: Identify and respond to safeguarding concerns and manage complex risk situations.
- Multi-Agency Working: Work collaboratively with families, carers, and partner agencies to provide holistic support.
- Documentation: Maintain accurate records and reports using systems such as LAS.
- Transition Support: Ensure seamless transitions that promote independence and positive life outcomes.
