Adult Transitions Social Worker — Hybrid Role

Liquidpersonnel is looking for an Adult Social Worker to join the Young People Transitions to Adulthood Team based in Oldham. The role involves assisting young individuals as they transition from children’s to adult services by providing comprehensive support and coordinating multi-agency transition plans.

Candidates will help promote independence, develop life skills, and ensure a seamless transition for young people. This position offers a hybrid work model and initial 3-month contract along with supervision and training.
