Job Purpose
As a Social Worker within the Children with Disabilities Team, you will support children and young people with disabilities and their families to achieve the best possible outcomes. You will carry out holistic assessments, develop and coordinate tailored support plans, and work collaboratively with families and partner agencies to safeguard children, promote their wellbeing, and help them reach their full potential. Using a restorative and relationship-based approach, you will ensure the voice of the child remains central to all decision-making while delivering high-quality, person-centred social work practice.
Key Responsibilities
- Complete statutory assessments, care planning, reviews, and interventions for children and young people with disabilities.
- Build strong, trusting relationships with children, young people, and their families to understand their individual strengths, needs, and aspirations.
- Develop and coordinate effective support plans that promote independence, inclusion, and positive outcomes.
- Work collaboratively with professionals across health, education, and social care to deliver coordinated, multi-agency support.
- Safeguard children by identifying risks, implementing appropriate interventions, and ensuring statutory responsibilities are met.
- Advocate for children and young people, ensuring their views, wishes, and experiences are central to assessment and planning.
- Maintain accurate, timely, and high-quality case records in line with legislation, policy, and professional standards.
- Contribute to a restorative, reflective team culture, participating in supervision, professional development, and continuous service improvement.
Requirements
- Qualified Social Worker with a recognised Social Work qualification (Degree, DipSW, CQSW or equivalent).
- Registered with Social Work England.
- Experience of undertaking assessments, care planning, and statutory social work interventions for children and families.
- Knowledge of legislation, guidance, and best practice relating to children with disabilities and safeguarding.
- Strong communication and relationship-building skills, with the ability to work effectively with children, families, and partner agencies.
- Ability to manage a varied caseload, prioritise effectively, and make sound professional decisions in complex situations.
- Commitment to relationship-based, child-centred practice and achieving positive outcomes for children and young people.
